Flushing and suction process needs to pay attention to the important matters and operation steps in detail, to ensure the smooth progress of the surgical process and the safety of patients.
First. Important matters
Aseptic operation:
The use of flushing suction device must be carried out in the operating room in accordance with the aseptic operation procedures.
When taking off the bag, the nurse should tear off the paper-plastic bag and PE bag from the handle position to ensure the sterility of the instrument.
Attachment of physiological saline bags:
The connection of the saline bag should be firm and reliable to avoid leakage during flushing.
Determine the capacity of the saline bag according to the needs of the operation, usually 500mL, if the operation time is long or bleeding, you can choose a larger specification and spare more than one.
Negative pressure suction device connection and adjustment:
Connect the connector of the flush suction device to the negative pressure suction device in the operating theatre and make sure the connection is tight.
Adjust the negative pressure to the appropriate range (e.g., 450 mmHg, 0.06 MPa) to avoid excessive negative pressure causing damage to the surrounding tissues or too little negative pressure affecting the suction effect.
Timing of flushing and suction:
Flushing and suctioning should be performed according to the progress of surgery and the need for cleaning the surgical field.
When the surgical area affects the operator's field of vision, flushing should be carried out immediately; suction should be carried out after the flushing is completed to ensure that the surgical field is clear.
Avoid excessive suction:
During suctioning, close attention should be paid to the suctioning of the fluid to avoid excessive suctioning leading to a reduction in pneumoperitoneum and affecting the continuity of the operation.
The suction button should be released immediately after the liquid is sucked dry to prevent suction injury to the surrounding tissues.
Second, the operation steps
Unpacking and preparation:
In the operating theatre, remove the irrigation suction device from the package according to the aseptic operation procedure.
The nurse tears open the paper-plastic bag and PE bag from the handle position, exposing the handle for the surgeon to take.
The saline bag is attached and flushed:
Tear off the connection tube of the irrigation suction device and puncture the bottle insertion needle into the saline bag.
Determine the volume of the saline bag according to the surgical needs and connect it to the flush suction device.
Open the stopcock and press the green button of the flushing valve to allow the saline to fill the entire line.
Connect and adjust the negative pressure suction device:
Connect the connection of the flush suction device to the negative pressure suction device in the operating theatre.
Adjust the negative pressure to the appropriate range and press the white button to check if the connection is successful (a ‘hissing’ sound of suction air is heard).
Flushing operation:
When the surgical area needs to be flushed, aim the metal flushing suction tube at the area.
Press the green button to flush and the saline will flow towards the surgical area using natural gravity.
When the operator believes that the area has been flushed clean, release the green button to stop flushing.
Suction operation:
After flushing, extend the metal tube below the fluid level.
Press the button for suction to draw the fluid away quickly.
When the liquid is all suctioned clean, release the button to stop suction.
Be careful to avoid excessive suctioning to prevent pneumoperitoneum reduction.
Special operating techniques:
When suctioning a small amount of oozing blood or blood clot, the operation of quick suction and quick return can be used to avoid the decrease of abdominal pressure caused by continuous suction.
For smaller bleeding points, the ‘point suction’ operation can be used, that is, the metal tube mouth on the blood point quickly press the suction button.
In summary, the single-use irrigation suction device has a wide range of application value in the surgical process. In the process of using, must strictly abide by the aseptic operating procedures, correctly connect the saline bag and negative pressure suction, and master the rinsing and suction of the operating steps and techniques. Only in this way can we ensure the smooth operation and patient's safety.
